TDTU notices on Applying for Health Declaration Form Online

In an attempt to ensure safety against the Covid-19 pandemic, Ton Duc Thang University requires all lecturers, officers, and students to make a medical declaration daily on the Lecturer/Student Portal. Visiting lectures, visitors please make a medical declaration on your mobile device before entering the school.

Step 2:

Select “Graduates” to make a survey

Step 3:

Tick Yes/No in the survey. Make sure you read it carefully.

Step 4:

Click “I am not a machine”, then click “Send”

Step 5:

Please capture your screen and show it to security before entering the school.
